Description of innovation
We can provide you domiciliary care including personal care and moving & handling, as well as provide you over-the-counter and pharmacy medicines and private prescriptions. We can also review your medicines use and contact GP surgeries and other healthcare units regarding management of your appointments and prescriptions, to expedite social care assessments .
This service aims to provide a better integration between health and social care services leading to better outcomes for patients and service users. Also as these services are provided by self-employed pharmacists and self-employed carers or self-employed social workers (directly employed by an individual, or related third party, or with an agreement to work directly for an individual) , this will be cheaper than care home or standard domiciliary agencies services and carers/pharmacists/ social workers also will earn more.
Each carer/pharmacist/social worker will be responsible to present their qualifications to each of their clients and agree care plans with their clients. Each carer/ pharmacist/ social worker will contribute with a small percentage to this social enterprise as well as clients will be able to donate for us to help other clients in need.

I am registered pharmacist (GPHC registration no. 2206096) and a carer (Social Care Wales registration no. W/5131135) with more than 8 years experience working in the UK. I can do moving and handling, personal care and monitor medication treatments. I have experience with Parkinson's disease, Alzheimer's, multiple sclerosis, cerebral palsy, learning disabilities, Down syndrome, elderly. My care plans:

Poor quality of Social Care provision and of its interaction with Health Care/ NHS. Domiciliary care and care home agencies service quality is very poor and very expensive. Pharmacies management of prescriptions is disastrous
